OPH will be holding an immunization clinics for intermediate students May 4th. Location in the school is still to be determined. Only students who complete a consent form will be allowed to get their necessary immunizations. Attached are two documents that provide more information as to how the clinics will be run.
Please view the attached documents to determine if your child is missing any of the required immunizations. If your child requires vaccination please fill out the consent form to have it ready at the time of the vaccination.
